Chapter 12: Coronavirus CPR Guidelines
In April 2020, the American Heart Association (AHA) released updated CPR guidance to address the realities of the COVID-19 pandemic and the anticipated rise in cardiac arrests. These interim guidelines aim to:
- Limit the number of responders in direct contact with the patient
- Reduce risk of exposure for healthcare providers
- Encourage bystander CPR—especially hands-only chest compressions
Healthcare Provider Adult CPR Update
Healthcare workers are often closest to patients who may be suspected or confirmed to have COVID-19—and as a result, are among those at highest risk of infection. These updates are designed to support safety while continuing effective care:
- Put on PPE before entering the scene.
- Begin 30:2 CPR cycles—30 compressions followed by 2 rescue breaths.
- Use a bag-mask device with a tight seal and filter when giving breaths, or perform continuous compressions with passive oxygenation using a mask.
- Use mechanical CPR devices when appropriate to minimize aerosol risk.
Chest Compressions for Adults
- Perform at least hands-only CPR if a cardiac arrest is identified.
- Use a face mask or cloth to cover the rescuer’s and/or victim’s mouth and nose to help lower the chance of virus transmission, especially between non-household individuals
Chest Compressions for Children
- Perform compressions and consider mouth-to-mouth ventilation
- A face mask or cloth covering the mouth and nose of the healthcare provider and/or the victim may help reduce the risk of COVID-19 transmission to a non-household bystander.
